Hospitals try to lower readmission rates

A local health care organization is trying to make sure patients don't have to make another hospital visit, after they're released.

A study shows the readmission rate is around 15 percent for medical patients, and 10 percent for surgical patients in Appleton and Neenah.

ThedaCare patient transitions vice president Jodi Braun says hospital officials talk about a discharge plan as soon as a patient checks-in. She says they work on it throughout their stay, and they also run a "return risk assessment." She says if they have any risk factors, they make sure to follow up with a phone call, to ensure they're following through with their plan.

Braun says the effort is paying off. She says in the last couple of years, the readmission rate for heart failure patients dropped by more than two percent.
